If you’re tired of dieting and working out and still not seeing smoothing and shaping results in certain areas of your body, you’re not alone. Many people just like you are fed up with a round belly, jiggly arms and buttocks. If fat reduction surgery is not for you, DeSilva Dermatology has good news.

We offer two of the most effective, non-surgical body sculpting procedures available today. Read on to find out which one may be right for you.

truSculpt ID

truSculpt ID® utilizes monopolar radiofrequency energy to gently heat fat cells so they can be flushed from the body. It is effective on many different fatty areas, such as:

the stomach

the flanks

the buttocks

Each session takes about 20 minutes. Based on a free body consultation, we will recommend how many treatments will be required to achieve the results you want. You can even treat more than one area at a time. And there’s no downtime after each procedure. You can return to your regular schedule immediately.

truSculpt ID procedures are typically described as feeling like a hot stone massage. During treatment, the temperature of your skin will rise but remain comfortable. It’s effective and safe — and the results are even and consistent.

EMSCULPT®

EMSCULPT® uses high-frequency electromagnetic energy to cause muscle contractions beyond what the body can achieve on its own. The result is intense fat burning and muscle building at the same time. EMSCULPT® can be used on problem areas such as:

the stomach

the buttocks

the arms

The procedure feels like an intense workout — but one that you can lay down and relax during!

It will give you a toned, sculpted look with just a few treatments. There’s no downtime afterwards, and you can return to your regular schedule immediately. Most patients benefit from a package of treatments spaced every few days for a few weeks, during which time you’ll see results like greater muscle mass and less fat. It’s been FDA-approved for five different body parts and is most effective on the abdomen, buttocks and arms.

We can customize your treatment package to meet your fitness goals and return for enhancement treatments as needed.

Which Treatment Is Best for You?

The most important thing to consider when choosing between truSculpt ID® and EMSCULPT® are your body sculpting goals.

truSculpt ID® is optimal for fat reduction and, as an added benefit, provides some mild skin contraction.

EMSCULPT® is recommended for patients who want to build muscle mass, strengthen their core and achieve definition.

During your free body contouring consultation, we’ll help you choose which treatment is right for you. Or, a combination of both treatments may help you achieve optimal results.

Sun-damaged and aging skin thins the epidermis and dermis, along with decreasing collagen levels, resulting in fine lines and wrinkles as well as tone and texture changes and sagging skin. Microneedling involves damaging the skin to encourage an increase in collagen was well as new dermal tissue. This results in a fuller-looking appearance. Rather than affecting the entire surface, RF causes thermal damage to the dermis in much smaller areas.
Fractional RF microneedling is a minimally invasive treatment that involves the use of very fine sterilized needles to create microscopic wounds at various depths in the dermis — without affecting the epidermis.

Seborrheic keratosis is a common skin formation that can initially cause concern because of its tendency to look like a pre-cancerous skin growth or skin cancer. In fact, these growths are benign.

Since it usually affects people who are middle-aged or older, seborrheic keratosis also goes by other names, such as wisdom spots and barnacles. It can appear on people with all skin types. It’s also hereditary. Some women can develop seborrheic keratosis when pregnant or after hormone replacement therapy.

Beginning as a small bump, seborrheic keratosis gradually thickens and becomes wartlike. The colors can include: brown, gray, white, pink or black, but all have a classic waxy scale. Sizes range from less than half an inch to two inches. It commonly appears on the back, stomach, scalp, face and neck or any other part of the body — except for the palms and the soles of the foot. It may also appear smaller around the eyes.

Seborrheic keratoses are usually painless, but since they are raised, they can be snagged on clothing and traumatized, causing them to swell, scale up, and become itchy. This does not convert them to anything dangerous; just irritated, at which time they can be treated with cryotherapy (freezing and destroying the growth).

“Make sure to have any suspicious bumps examined by your dermatologist,” says DeSilva Dermatology’s Dr. Thushan DeSilva. “We want to make sure that it’s a seborrheic keratosis and not skin cancer.” A dermatologist can usually diagnose seborrheic keratosis visually and enhanced by dermoscopy (a hand-held magnifier) to minimize the need to biopsy.

If you find new, suspicious growths on your skin, contact DeSilva Dermatology at (830) 331-4150 for an appointment.

DeSilva Dermatology now offers EMSCULPT, the next generation in non-invasive body sculpting techniques. Here are the answers to questions you might have about this revolutionary treatment.

How does EMSCULPT work?

The non-invasive HIFEM® procedure induces powerful muscle contractions not achievable through voluntary contractions.

When exposed to these strong contractions, the muscle tissue is forced to adapt to such an extreme condition. It responds with a deep remodeling of its inner structure that results in muscle building and sculpting of your body.

What areas of the body does EMSCULPT treat?

In addition to the abdomen and buttocks, EMSCULPT can also tone legs and arms and provide desired definition.

Am I a candidate?

Yes, everyone can be a candidate for EMSCULPT. Results will vary depending on a patient’s goals and response to the treatment.

What is the treatment time?

A 30 minute treatment with minimum of 4 sessions scheduled two to three days apart is recommended. However, DeSilva Dermatology can help you create a treatment plan tailored to your specific goals.

What does it feel like?

The procedure feels like an intense workout — but one that you can lay down and relax during!

Is it safe?

Yes! EMSCULPT has been FDA-cleared for treatments described above.

Is there any downtime or pre/post treatment preparation?

Since the EmSculpt is non-invasive, there is no downtime or need for pre/post treatment preparation.

How quickly will I see results?

You will feel tangible results immediately after the treatment. Positive results are usually reported two to four weeks after treatment and continue to improve for several weeks.

Be one of the first people in Boerne to get the EMSCULPT advantage. Schedule a consultation with DeSilva Dermatology today!

How does Excel V work?

Excel V is a laser system for providing treatment of a variety of vascular skin conditions. Many skin conditions manifest in the form of broken or abnormal blood vessels in the skin. Such is the case with rosacea, in which the facial blood vessels dilate excessively, causing redness and flushing. Excel V treats these conditions by administering a unique high-power green laser that is absorbed by the abnormal blood vessels in the skin. The laser heats the abnormal vessels until they are removed. At the end of treatment, the patient’s natural skin tone and color are restored.

What specific conditions does Excel V treat?

Excel V has been shown effective at treating a number of conditions, including rosacea, telangiectasias, leg veins, port wine stains, and cherry angiomas. Excel V treats mild and severe forms of these conditions. The system may also have a cosmetic benefit for patients who have skin lesions or scars from past injuries or surgeries.

What are the benefits of Excel V compared with other treatment options?

The benefit of Excel V is that the treatment can be highly adjusted to ensure that the level of treatment being delivered specifically matches the patient’s skin condition. The system contains various wavelength modes, allowing the delivery of both short pulses for superficial blemishes, such as small lesions and skin discoloration, and longer pulses for deeper vascular conditions, such as leg veins and nodular port wine stains. It also offers a wide range of laser spot sizes to ensure that small treatment areas can be targeted at greater precision and larger areas can be treated in a shorter period.

Are there other laser treatment options available for these skin conditions?

Yes, there are other laser treatment options available for skin conditions. The challenge usually associated with laser treatment is tailoring the treatment to precisely match the patient’s unique skin condition. Some existing lasers may be effective for treating a condition at one size and depth, but may not work effectively on conditions that affect a larger area.

What is the level of demand for treatment of skin conditions?

The prevalence of vascular skin conditions varies greatly. Rosacea is one of the most common conditions, affecting about 14 million Americans. Many people with Rosacea may not even realize they have it even when common symptoms are present such as redness in the face and flushing after consumption of alcohol, caffeine or spicy foods.

Another fairly common condition is cherry angiomas, which are small red bumps that spontaneously appear on the skin. These kind of lesions are commonly found in middle‐aged populations—more than 85% of people over 45 years of age have several cherry angiomas.

Spider veins are also a fairly common condition, although the exact prevalence varies according to different sources. Port wine stains, another condition which Excel V treats, is a more rare condition, occurring in about three out of every 1,000 people. These are a type of birthmark often appearing on the face in which swollen blood vessels cause a reddish‐purplish discoloration of the skin.

Although these conditions represent some of the most common cases for which the Excel V laser system provide treatment, the robust customization of the system makes it ideal for also treating more general cosmetic problems such as superficial blemishes, discoloration and even scars. Given that the large majority of people have one or more of these conditions to varying degrees, almost anyone could potentially benefit from treatment with Excel V.

How is the procedure performed?

After an initial consultation with the patient, we will determine if treatment with the Excel V is appropriate.

Unlike other procedures, injections or topical anesthesia are not required throughout the duration of the 15-30 minute procedure. Depending on the condition, the Excel V hand piece is placed over the affected areas of the skin until the abnormal vessels diminish. To minimize discomfort and protect the patient’s skin, the Excel V uses a proprietary cooling mechanism

What skin types does Excel V treat?

All skin types identified in the Fitzpatrick skin type classification can be treated with the Excel V system. This classification denotes 6 different skin types, ranging from very light (type I) to very dark (type VI). The 532nm wavelength is ideal for treating skin types I-IV and the 1064nm mode can be used on all skin types I-VI.

How many treatments are needed?

The number of sessions depends on various parameters, including the size, thickness and color of the vascular lesion. Superficial vascular lesions such as small telangiectasia and diffused redness are easiest to treat; some patients might see significant improvement after just one treatment.

What are the costs of treatment?

As with any procedure, the costs depend on the specific condition for which treatment is being provided. For minor conditions only requiring a single cycle of treatment, the cost generally ranges between $250‐$1,000. For conditions that affect a large portion of the skin and require additional treatment sessions, the price increases proportionally.

To find out if Excel V is the right treatment for you, schedule a consultation with DeSilva Dermatology.

As you get older, there are many ways your skin can show signs of age:

It becomes thinner, loses fat, and no longer looks as smooth as it did.

Your veins and bones can be seen more easily.

Scratches, cuts, or bumps can take longer to heal.

Years of tanning or being out in the sunlight for a long time may lead to wrinkles, dryness, age spots and even cancer.

However, there are things you can do to protect your skin and to make it feel and look better. Here are some important tips:

Dry, Itching Skin

Many older people suffer from dry spots on their skin. It’s often found on their lower legs, elbows, and lower arms. These dry patches can feel rough and scaly. Some of the possible reasons for dry skin are:

Not drinking enough liquids

Spending too much time in the sun or tanning

Being in very dry air

Smoking

Feeling stress

A reduction in sweat and oil glands, which is common with age

Dry skin also can be caused by:

Health problems such as diabetes or kidney disease

Using too much soap, antiperspirant or perfume

Taking too many hot baths

Medications

Because older people have thinner skin, scratching can cause bleeding that can lead to infections. Talk to your doctor if your skin is very dry and itchy.

Here are some ways to help dry, itchy skin:

Use moisturizers, like lotions, creams, or ointments every day.

Take fewer baths or showers and use milder soap. Warm water is less drying than hot water.

Try using a humidifier to add moisture to your room.

Bruises

Older people may bruise more easily than younger people, and it can take longer for these bruises to heal. Some medicines or illnesses may also cause bruising. Talk to your doctor if you see bruises and don’t know how you got them, especially on parts of your body usually covered by clothing.

Wrinkles

Over time, skin begins to wrinkle due to environmental effects. Ultraviolet (UV) light from the sun can make the skin less elastic. Gravity can cause it to sag and wrinkle. Certain habits, like smoking, can also wrinkle the skin.

If you want to reduce wrinkles, talk to a board-certified dermatologist about possible treatments.

Age Spots and Skin Tags

Age spots, once known as “liver spots,” are flat, brown areas often caused by years in the sun. They are bigger than freckles and commonly show up on the face, hands, arms, back and feet. Using a broad-spectrum sunscreen that helps protect against two types of the sun’s rays (UVA and UVB) may prevent more age spots.

Skin tags are small, usually flesh-colored growths of skin that have a raised surface. They become common as people age, especially for women. They are most often found on the eyelids, neck, and body folds such as the armpit, chest and groin.

Age spots and skin tags are typically harmless, although sometimes skin tags can become irritated. If your age spots or skin tags bother you, talk to your dermatologist about having them removed.

Skin Cancer

Skin cancer is one of the most common types of cancer. The main cause of skin cancer is the sun, but artificial sunlight such as sunlamps and tanning booths can also cause skin cancer. Anyone of any skin color can get skin cancer, but people with fair skin that freckles easily are at greatest risk. Skin cancer may be cured if it is found before it spreads to other parts of the body.

There are three types of skin cancers. Two types, basal cell carcinoma and squamous cell carcinoma, grow slowly and rarely spread to other parts of the body. These types of cancer are usually found on parts of the skin most often exposed to the sun, like the head, face, neck, hands and arms. However, they can happen anywhere on your body. The third and most dangerous type of skin cancer is melanoma. It is rarer than the other types, but it can spread to other organs and be deadly.

Since skin cancer is rarely painful, you should check your skin once a month for things that may be signs of cancer. Look for changes such as a new growth, a sore that doesn’t heal or a bleeding mole.

Check Moles, Birthmarks, or Other Parts of the Skin for the “ABCDE’s”

A = Asymmetry (one half of the growth looks different from the other half)

B = Borders that are irregular

C = Color changes or more than one color

D = Diameter greater than the size of a pencil eraser

E = Evolving; this means the growth changes in size, shape, symptoms (itching, tenderness), surface (especially bleeding), or shades of color

See your doctor right away if you have any of these signs to make sure it is not skin cancer.

Keep Your Skin Healthy

Getting some sun is good for you, but to protect your skin, be sure to:

Limit time in the sun. It’s okay to go out during the day, but try to avoid peak times when the sun’s rays are strongest. During the summer, try to stay out of the sun between 10 a.m. and 4 p.m. Don’t be fooled by cloudy skies — the sun’s rays can penetrate clouds. You can also get sunburned if you are in water, so be careful when you are in a pool, lake, or the ocean.

Use sunscreen. Look for sunscreen with an SPF (sun protection factor) number of 15 or higher. It’s best to choose sunscreens with “broad spectrum” on the label. Put the sunscreen on 15 to 30 minutes before you go outside. Sunscreen should be reapplied at least every two hours. You need to put sunscreen on more often if you are swimming, sweating, or rubbing your skin with a towel.

Wear protective clothing. A hat with a wide brim can shade your neck, ears, eyes and head. Look for sunglasses that block 99 to 100 percent of the sun’s rays. If you must be in the sun, wear loose, lightweight, long-sleeved shirts and long pants or skirts.

Avoid tanning. Don’t use sunlamps or tanning beds. Tanning pills are not approved by the U.S. Food and Drug Administration (FDA) and might not be safe.

Your skin is an organ that comes into contact with the rest of the world. It holds your body fluids in, prevents dehydration and keeps harmful microbes out of your system. Without it, we would get infections.

Your skin is full of nerve endings that help you feel things like heat, cold and pain. If you couldn’t feel these things, you could get badly hurt and not even know it!

Why is healthy skin important?

Since your skin plays such an important role in protecting your body, you should keep it as healthy as you can. This will help you keep from getting sick or having damage to your bones, muscles and internal organs.

What you might not know about your skin

Skin is actually your body’s largest organ by size. Your skin helps keep your body temperature even. If you get too hot, blood vessels near the surface of the skin, called capillaries, enlarge to let the warm blood cool down.

Your skin also makes vitamin D when the sun shines on it. Vitamin D is important for the health of your bones and other parts of your body.

Keep it clean

Washing your skin, especially your hands, is very important for keeping it healthy. Hand washing keeps you from spreading germs to other parts of your body. It also keeps you from spreading germs that could give others a cold or the flu. To help keep your skin from getting dry, use water that’s comfortably warm, not too hot, when you take a bath or shower. If your skin is dry or itchy, try a moisturizing cream or lotion.

Enjoy the sun — but protect your skin

Your skin produces vitamin D when it is exposed to sunshine. Vitamin D helps keep your bones and other body systems healthy. However, too much sun can damage your skin and increase your risk for skin cancer. It may make your skin look old years too soon and can make it less able to fight off infections.

Whenever you’re outside, use a sunscreen with a sun protection factor (SPF) of 30 or 45 as well as zinc oxide as the main ingredient. Apply it evenly, and have a friend or parent help you with the hard-to-reach spots. Follow the directions that tell you how often to reapply it — one application won’t last all day!

What about tanning beds?

Tanning beds don’t offer a safe alternative to natural sunlight. Exposure to ultraviolet (UV) radiation damages your skin, whether the exposure comes from tanning beds or natural sunlight.

This damage increases the risk of skin cancer and premature skin aging, just like too much sun will. In fact, most tanning beds emit mainly UVA rays, which may increase the risk of melanoma, the deadliest form of skin cancer.

Physical activity

Being physically active is good for your skin! It increases the flow of blood to the surface of your skin and brings oxygen and nutrients to your whole body. Sweating helps to flush out impurities from your skin. Get one hour or more of physical activity every day. This will be good for your skin as well as your heart, lungs, muscles and other parts of your body.

Be sure to drink enough water to replace the fluids you lose when you sweat. If you have any concerns about your health, talk to your doctor or a physical therapist to find out what kinds of activities are right for you.

A healthy diet

You really don’t need a special diet to keep your skin in good health. Eating a balanced diet will help you maintain a healthy weight and provide a variety of nutrients for your skin and your overall health. A balanced diet:

Consists of fruits, vegetables, whole grains, and fat-free or low-fat dairy products like milk, cheese and yogurt.

Includes protein from lean meats, poultry, seafood, beans, eggs and nuts.

Is low in solid fats, saturated fats, cholesterol, salt (sodium), added sugars and refined grains.

Is as low as possible in trans fats.

Balances calories taken in through food with calories burned in physical activity to help maintain a healthy weight.

Get sufficient sleep

Getting enough sleep helps improve your overall health, which is good for your skin. Teens need at least nine hours of sleep each night, while adults need about seven to nine hours.

See your doctor

If you find anything unusual on your skin, like a mole that changes size or color or a patch that looks red or itches, make an appointment. For skin diseases, it’s important to see a doctor as early as possible to prevent permanent damage.

Start now

Healthy skin will help you look your best and feel good about how you look.

“At DeSilva Dermatology, we offer the services and products you need to keep your skin looking beautiful for a lifetime,” says Dr. Thushan DeSilva. “It’s never too early to start a healthy skin regimen.”

What is Secret RF™?

Secret RF is a novel approach to deliver fractional radio frequency (RF) energy to all layers of the skin with minimal damage to the skin’s surface. The multi-layered treatment is tailored to each individual and addresses their unique skin concern during each customized treatment.

What skin conditions does it treat?

The Secret RF treatment is ideal for improving aging skin, photodamage, fine lines and wrinkles on the face and neck, scars, acne scars, skin quality and stretch marks. The treatment is safe, effective and suitable for all skin types in patients who seek skin revitalization.

How does it work?

Secret RF uses microneedles to deliver RF energy into varying levels of the skin including the deeper layers where treatment is most beneficial. Unlike other devices which only deposit energy in upper layer of the skin, Secret RF delivers RF energy below the surface where it’s needed most to help revitalize and regenerate the tissue. This unique delivery of energy allows us to achieve optimal results with little to no downtime on all skin types.

What is the Secret RF procedure like?

Secret RF is an easy, safe and effective procedure that can be performed in a quick, 15-minute treatment session. During the treatment, you will feel slight warmth and some pressure on the treatment area. For most patients, the treatment is tolerable, but your physician will work with you to maximize your comfort level. Prior to the procedure, your provider will apply a numbing cream to the treatment area to increase comfort during the treatment.

How many Secret RF treatments will I need?

Patients typically need 3-4 treatments spaced 4-6 weeks apart to achieve optimal results. Most patients report seeing improvement after the second treatment. However, results are optimized three months after the last treatment.

How long does a treatment last?

Results vary based on the individual. To maintain results, maintenance treatments are recommended every 12-18 months.

What can I expect after my procedure?

After the treatment, an ointment will be applied to the skin and you will be asked to refrain from wearing makeup for several hours. Any minor redness or swelling resulting from treatment will subside within 12-24 hours. After the treatment, you can expect to return to normal activities.

“SECRET RF™ is another one of the latest technologies that our practice is the first to offer in the Boerne area,” said DeSilva Dermatology’s Dr. Thushan DeSilva. “We think our patients will be thrilled with how tolerable this effective procedure is.”

What is truSculpt® iD?

truSculpt iD allows you to personalize your ideal body with a revolutionary non-surgical, body sculpting treatment. It’s comfortable, safe and effective. This technology is clinically proven to permanently eliminate fat cells in stubborn areas around your abdomen and flanks.

Am I a candidate for truSculpt iD?

Because truSculpt iD is a customized and tailored treatment to meet individual needs, most adult men and women are candidates. We recommend a consultation with one of our trained staff members to determine the optimal treatment that’s right for you.

Can truSculpt iD be used on all skin types?

Yes, this treatment can be done on all skin types and over tattoos.

How long does a treatment take?

Procedure time depends on the size and number of treatment areas. It typically takes 15 to 30 minutes to treat the abdomen and flanks.

How many treatments will be needed?

Multiple areas can be treated in as little as one 15-minute session. Additional treatments may be required to achieve the personalized results you desire.

What will the treatment feel like?

truSculpt iD procedures are typically described as feeling like a hot stone massage. During treatment, the temperature of your skin will rise but remain comfortable. It’s effective and safe — and the results are even and consistent.

Is there really no downtime?

Post-procedure, the treatment area will be warm and flushed for less than one hour. While you may experience mild sensitivity to the touch for a few days, you’ll be able to immediately return to normal activities and exercise.

When will I see results?

Results are visible after just one treatment, with maximum results seen after 12 weeks. Additional treatments may be required to achieve the personalized results you desire.

“truSculpt® iD is one of the newest and most effective body sculpting treatments,” said DeSilva Dermatology’s Dr. Thushan DeSilva. “We’re excited to be the first practice in the Boerne area to offer it.”
